On 17 January 1966 a United States Air Force B-52G collided with a KC-135 tanker during mid-air refueling at 31,000 feet over the Mediterranean Sea near Palomares in Almeria province in Spain. The collision destroyed the tanker and killed all four crew members, and the bomber broke apart with three of its seven crew members killed. If you fly refueling in a simulator, you can read this accident as a lesson in closure, position and breakaway discipline. The B-52G was on Operation Chrome Dome, a Cold War airborne alert with nuclear-armed bombers, and its flight plan took it east across the Atlantic Ocean and Mediterranean Sea toward the European borders of the Soviet Union before returning home. That long profile required two mid-air refuelings over Spain, so the second contact was not an extra but part of the plan.
The flying boom is a rigid telescoping tube with movable flight control surfaces that a boom operator extends and inserts into a receptacle on the receiver. You can review how the rigid boom works before you practice formation, because the steps are fixed. The tanker and receiver rendezvous and fly in formation, then you as receiver move behind the tanker within safe travel limits for the boom, helped by director lights or by directions radioed by the boom operator. Once in position the operator extends the boom to make contact, and fuel is pumped through the boom when the nozzle mates with the receptacle. A poppet valve in the nozzle keeps fuel from leaving the tube until mating is correct, and toggles in the receptacle hold the nozzle locked during transfer. When the planned offload is done, both aircraft disconnect and you depart the formation.
While in contact you must keep flying inside the air refueling envelope, the volume where contact with the boom stays safe. Move outside that volume and you can damage the boom or cause a mid-air collision, and the matter names the 1966 Palomares crash as the example. The boom is attached at the rear of the tanker on a gimbal, so it can move with you, and the small airfoils in V-tail arrangement move it by aerodynamic force under hydraulic actuation from the operator control stick. The operator also telescopes the tube to keep the connection. If you drift toward the outer limits, the operator tells you to correct and disconnects the boom if needed. When not in use the boom is stored flush with the bottom of the fuselage to reduce drag. In the KC-97 and KC-135 the operator lies prone and watches through a window at the tail, while in the KC-10 the operator is seated and in the KC-46 two operators sit at the front and watch camera video on 3D screens.
The probe-and-drogue method uses a flexible hose that trails from the tanker with a drogue or basket at the end, shaped like a shuttlecock, and a hose drum unit that reels the hose when not in use. You fly your rigid probe, mounted on the nose or fuselage and often retractable, into the basket, and valves open only after mating. You can compare this with how the hose and basket work to feel the difference from boom flying. The NATO standard probe uses shear rivets that attach the fuel valve to the probe tip, so a large side or vertical load shears the rivets and drops the valve instead of breaking the probe or the airframe. A so called broken probe can follow poor technique or turbulence, and the valve can stay in the drogue and block that hose until ground maintenance removes it. Your best defense is a stable approach from behind and below at a closure near two knots, because too little closure gives no flow and too much closure can set the hose oscillating and cut the tip.
At about 10:30 am the B-52G started its second refueling with a KC-135 out of Moron Air Base in southern Spain. Pilot Major Larry G. Messinger later recalled that the bomber came in behind the tanker a little fast and started to overrun it, that no break away call came from the boom operator, and that the situation then broke apart very fast. The nozzle of the boom struck the top of the B-52 fuselage, broke a longeron and snapped off the left wing, and a second B-52 about a mile away saw the explosion. The detailed crew list and timeline in this account of the Palomares collision show how fast escape options close once metal touches. On the KC-135, boom operator Master Sergeant Lloyd Potolicchio, pilot Major Emil J. Chapla, co-pilot Captain Paul R. Lane and navigator Captain Leo E. Simmons were killed. On the B-52G, navigator First Lieutenant Steven G. Montanus, electronic warfare officer First Lieutenant George J. Glessner and gunner Technical Sergeant Ronald P. Snyder were killed, while pilot Major Messinger, aircraft commander Captain Charles F. Wendorf, co-pilot First Lieutenant Michael J. Rooney and radar navigator Captain Ivens Buchanan parachuted to safety.
The B-52G carried four B28FI Mod 2 Y1 thermonuclear bombs. The letters FI mean full fuzing internal configuration, which in the matter covers free fall airburst, retarded airburst, free fall groundburst and laydown groundburst delivery, with a W28 warhead set between a Mk28 Mod 3F shock absorbing nose and a Mk28 Mod 0 rear end with a parachute. Mod 2 is the hardened version built to survive laydown delivery, where earlier Mod 0 and Mod 1 weapons could not survive the forces, and Y1 marks a yield of 1.1 megatonnes of TNT. Three weapons were located on land near Palomares within 24 hours of the accident, the conventional explosives in two detonated on impact without a nuclear explosion and spread plutonium over about 2 square kilometers, while the third was found relatively intact in a riverbed. The fourth weapon fell into the sea and was recovered only after an 80 day search with United States Navy ships and submersibles such as DSV Alvin, after witness Francisco Simo Orts pointed to the entry point and the search team used Bayesian search theory to update grid probabilities. Under United States military terminology the event is a Broken Arrow, a nuclear weapons accident more serious than a Bent Spear incident, because weapons were lost and destroyed, two weapons detonated non nuclear on land, and radioactive contamination occurred.
Training repeats the same cues you read in the procedures. You hold formation, you watch the lights, you listen for break away, break away, break away, and you disconnect at once when you hear it. For hose flying you watch the status near the hose drum unit, where green shows flow, amber with a cutoff shows the hose pushed too far or not far enough, and red from the tanker pilot orders disengagement. Tension on the hose is balanced by a motor so the hose extends and retracts as you move fore and aft, but the drogue stays light and the bow wave of your nose can push it aside even in smooth air. After Palomares the United States and Spain removed about 1,750 tons of radioactive soil for disposal in the United States, then monitoring continued and a 2015 agreement set further remediation, yet as of 2025 some contaminated land remains. Spain banned United States flights carrying nuclear weapons over its territory, and Palomares with the 1968 Thule Air Base B-52 crash contributed to the end of Operation Chrome Dome. Buchanan was carried by Palomares residents to a local clinic, Wendorf and Rooney were picked up at sea by the fishing boat Dorita, and Messinger spent 45 minutes in the water before Francisco Simo brought him aboard the fishing boat Agustin y Rosa for transfer to a hospital in Aguilas.
